30 Day Autoship Program
I understand that by receiving a 30 Day Supply of Latavi at the discounted rate of $59.99 plus s/h I will be enrolled into the exclusive Autoship Program whereby I will automatically receive a 30 day supply at the Discounted Rate ($59.99 plus shipping/handling) 30 days after my original order date and every 30 days thereafter. I understand that I may cancel my membership to the Autoship Program at anytime prior to the refill, by contacting Customer Care. I understand that there is no obligation to remain in the Autoship Program, and you can discontinue future shipments at any time.

Collections
I agree not to charge back any orders I place with Shoreline Business Enterprises despite if I keep Latavi after the 30 day guarantee or are unhappy with the product. I understand that if I dispute any legitimate charges with my bank or credit card company that Shoreline Business Enterprises will send my account to collections. I agree to contact Customer Care to resolve any issues, rather than issue a chargeback with my credit card company.
